This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.2 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a separate written solicitation will not be issued. 1.a. Project Title: Equipment Transportation Services 1.b. Description: VA Greater Los Angeles Healthcare System located at 11301 Wilshire Blvd; Los Angeles CA 90073 is seeking a contractor to motorized vehicle runs for dental and medical instruments between the VA Greater Los Angeles Healthcare System (VAGLAHS) West Los Angeles (WLA) Campus and selected remotely located clinical facilities 1.c. Project Location: VA Greater Los Angeles Healthcare System, 11301 Wilshire Blvd, Los Angeles, CA 90073 1.d. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26227Q0067. 1.e. Set Aside: This solicitation is issued as an SDVOSB set aside. 1.f. Applicable NAICS code: 423860 Transportation Equipment and Supplies (except Motor Vehicle) Merchant Wholesalers. 1.g. Small Business Size Standard: 175 employees 1.h. Type of Contract: Firm Fixed Price 1.i. Period of Performance: one year 1.j. Wage Determination applicable to this project: SCA WD 2015-5613 revision 34 1.k. This requirement, driven by VA Central Office and The Joint Commission (TJC) on Patient Safety, mandates that dental and medical instruments are no longer permitted to soak in solution overnight. It is imperative that all instruments are returned to the Sterile Processing Service (SPS) for sterilization on the same day they are used. To comply with this mandate, it is necessary to arrange motorized vehicle runs between the VA Greater Los Angeles Healthcare System (VAGLAHS) West Los Angeles (WLA) Campus and selected remotely located clinical facilities.
